What is the form for?

To tell us about any medical problems that you have, which are due to, or made worse by, where you live. It also helps us understand your housing needs and decide if you should get any more Housing Register points.

Who should use the form?

Anyone who thinks that where they live is having an effect on their health or the health of someone who lives with them.

Who does not need to use the form?

Someone who needs to move, in order to give or to receive support from a relative.

Someone who has a medical problem that is not due to, or made worse by, where they live.

What if more than one person has a medical problem?

You should complete a separate form for each person with a medical problem.

Who does the assessment?

A qualified doctor who meets with our Rehousing and Advice Manager.

How often are the assessments done?

Once every three to four weeks.

What does the doctor decide?

The doctor decides if any more Housing Register points should be given to help someone move home. He does this by seeing if the medical problem is affected by where someone lives. If it is, the doctor will award 5,10,15,20 or 25 Housing Register points.

What happens if I don’t fully complete the form?

We check all forms before showing them to the doctor. If there is not enough information we will ask you to provide more details.

What if my home is suitable, but I don’t like the area I live in?

The doctor can look at this if the area you live in is affecting your health. It will help if you can provide information to support your case. For example, if you are being harassed, you may be able to give details about how the police have been involved.
